 脂肪肝 多因痰、瘀積聚肝臟形成

:

中醫師劉佳祐表示，脂肪肝顧名思義是肝臟囤積了太多的脂肪，中醫認為，脂肪肝是痰、瘀積聚在肝臟的結果，「痰」是指病理性的代謝產物，例如過度飲酒、飲食過多或過食油膩重口味或是人體因疾病（如糖尿病）導致體內脂肪代謝異常，或是不當的節食，都可能造成脂肪在肝臟中不正常的堆積，進而導致肝臟血流障礙，進而出現「瘀」的現象，接下來就會使得肝臟更加無法代謝體內的脂肪，使得「痰」與「瘀」互結，造成病情更趨惡化。

劉佳祐說明，中醫治療脂肪肝的方式多以疏利肝膽、健脾化濕的藥物為主，另外像是山楂、荷葉、丹參、茵陳蒿、川七、何首烏、澤瀉、決明子、大黃等中藥，也都對體內脂肪的代謝清除有直接或間接的幫助。

劉佳祐醫師表示，脂肪肝可以分為酒精性脂肪肝與非酒精性脂肪肝，若是民眾因長期大量飲酒造成的脂肪肝，就被稱為酒精性脂肪肝，容易演變為酒精性肝炎、肝硬化，進而可能導致肝癌等嚴重問題。

非酒精性脂肪肝則屬於一種代謝性疾病，通常與糖尿病、中央肥胖、高血脂等代謝綜合症的關係十分密切，經常一起出現，也會互相增加併發症的風險。

劉佳祐指出，脂肪肝可分為肝胃不和、肝膽濕熱及脾虛濕盛三型，肝胃不和型常見於壓力過大的上班族，可見胸口鬱悶，右上腹悶痛，容易噁心、腹脹，胃口不佳，即是肝氣鬱滯的表現，可用疏肝理氣的四逆散或柴胡疏肝散來治療。

肝膽濕熱型常見於過度飲酒或愛吃生冷冰品的病患。可見到「濕」証，如疲倦、噁心、食慾不振、大便黏膩的症狀；「熱」証，如口乾口苦、口臭嘴破、煩躁易怒、小便黃的症狀，可用清利濕熱的甘露消毒丹或黃連溫膽湯來治療。

脾虛濕盛型濕在體內聚集會進而化生為痰，導致血脈不通，甚至出現肝功能異常升高的現象，所以祛瘀化痰是脂肪肝的治療重點。可用二陳湯、導痰湯、血府逐瘀湯等藥物來治療。

 完成調查不難 發掘真相很難 FBI查卡瓦諾可能鎖定2重點

聯邦調查局 (FBI)因大法官提名人卡瓦諾涉及性攻擊指控，重新對他做背景調查，調查可能鎖定兩大重點，包括卡瓦諾提出當年在月曆上某天標註的內容，以及福特在27日作證時指稱當時在場的所有人。

不過調查範圍有限，主要仰賴有關的人志願接受問話和提供文件。

「華爾街日報」與「華盛頓郵報」報導，根據福特作證內容，在1982年夏天她聲稱被卡瓦諾攻擊當晚，那天聚會除了她，至少還有四男一女，總共六人。當她被卡瓦諾推到聚會場所二樓的臥室意圖性侵時，在場的有卡瓦諾、他的同學賈吉(Mark Judge)，與她共三人。

樓下還有三人，分別是福特好友里藍．凱瑟(Leland Keyser)、一位男生PJ史密斯(P.J. Smyth)，及另一位想不起名字的男生。

至於卡瓦諾提出當年的月曆，他在1982年7月1日那天寫了這行字：「Go to Timmy’s for skis with Judge, Tom, P.J., Bernie and Squi」。

●不能申請搜索 不能強迫作證

一些前檢察官說，由於這不是刑事調查，因此FBI探員不能申請搜索令狀，或由大陪審團發傳票強迫證人作證或交出文件。證人和其他人能夠拒絕合作，雖然與FBI探員交談通常能夠促使對方說實話。

指控卡瓦諾性攻擊的克莉絲汀‧福特信誓旦旦的指陳事發經過，卡瓦諾則同樣堅決否認。

進行背景調查對FBI是家常便飯；卡瓦諾即指出，過去26年他六度通過FBI背景調查。

對聯邦司法職務提名人進行背景調查，通常鎖定他們的專業生活。一些前FBI官員說，探員必須對至少30名曾與提名人共事的人約談問話，包括法官、律師和司法人員。探員也會找這些人建議他們約談的任何人問話。

不過，FBI雖然擁有龐大資源，要發掘事實真相還是很難；畢竟事情發生在幾十年前，而且福特宣稱事發時在場的卡瓦諾好友賈吉否認有這種事情。事隔這麼久，有關記憶早就淡忘，文件或其他資料可能早就不在。而且並非每一個人都向FBI說實話。

FBI奉令在一周內完成調查，這對它並非難事。專家表示大部分背景調查只要幾天就能完成，尤其FBI可能把本案列為最高優先，動員全國各地探員參與約談問話和蒐證。

FBI對進行重大調查並迅速獲得結果經驗豐富。在發生恐怖攻擊或槍擊屠殺後，FBI往往幾天或數周內就完成數以百計問話，清查錄影、電腦和手機紀錄等數量極為龐大的證據。

●只做調查報告 不做決定建議

至於FBI的調查結果能不能獲得信任則是另一回事，尤其這是充滿強烈政治情緒的事件。FBI只負責調查蒐證和提出報告，不做決定或建議。

共和黨及民主黨勢必都會選擇性的利用調查結果加強自己的論點，使FBI探員擔心調查發現的任何真相會被政治所掩蓋。
